1. voucher redeemed by store or company: a voucher that entitles somebody to a discount, refund, or gift, typically issued as a sales promotion

2. order form: a printed form, e.g. in an advertisement, that may be filled in and returned to order a product or request information
|
3. form for payment by installments: a form or card showing the payment due on a specific date for something that is paid for on an installment plan. The card is returned with the payment.

4. certificate of interest on bond: a detachable part of a bond that indicates a date and the amount of interest paid on that date. The holder must present it in order to receive payment of the interest.

5. ticket in rationing system: a ticket issued under a rationing system that entitles somebody to an amount of a rationed item and that must be handed in in exchange for that item
